The only form by which a noteholder votes on the plan, usually sent by its custodian. It gives the holding and the vote, and the custodian confirms the holding. It must reach the information agent before the voting deadline. Explanatory Statement, Appendix 7 · Chapter 8 · see Blocked Notes, Voting deadline

Explained in Chapter 8, Classes, the Convening Hearing and the Plan Meetings, of Restructuring Plans.

How it comes up, in Chapter 8
A noteholder votes only by an account holder letter (Appendix 7). Most investors hold through a custodian, a firm that keeps securities for its clients, and the custodian sends the letter for them. The letter gives the holding and the vote, and the custodian confirms the holding.
